1. The coverage for Outpatient Services - Electrocauterization in the Terms of Reference is amended to read as follows:

   A. Outpatient Services

   Benefits	Coverage
   Electrocauterization of skin lesions such as plantar warts, periungal warts, filiform warts and molluscum, contagiosum, in any part of the body, except genital warts and condyloma acuminata, prescribed by an Accredited Physician/Specialist.	Up to PhP1,000/member/year Prescribed by an Accredited Physician/Specialist up to MBL/member/year; PhP1,000.00 per region per day, maximum of 2 regions per day (e.g. face- 4 lesions, neck – 4 lesions, chest-4 lesions; coverage will be face up to PhP1,000 and neck up to PhP1,000 – total of PhP2,000 only for the day) Frequency of visits for repeat cautery: minimum of 6 months from last cautery.

   All other provisions of the Invitation to Bid and Instructions to Bidders not inconsistent herewith shall remain valid and binding.

   
   
2. The PHILGEPS Registration requirement under ITB Clause 12.1 (a) (i) refers to Platinum Membership.
